General Patient Info

We accept most insurances including Workers’ Compensation, No-Fault and governmental subsidized health care plans (Medicare and Medicaid). Patients are seen by appointment only. If you need to reschedule or cancel an appointment, please call at least 24 hours in advance (883-0515).

At your time of appointment, please make sure you have the following:

Medical Patient:

• Insurance card
• Co-pays
• Medical Care of WNY must be listed as primary, either Robert Cotsen, M.D., Internal Medicine or Michael Calabrese, M.D., General Practice

Physical Therapy Patient:

• Insurance card
• Co-pays
• Referral script from primary care physician
• Referral script from health insurances company (if necessary)
